Financial Policy
L & M Physicians is committed to providing you the best possible care, and we are pleased to discuss our professional fees with you at any time. Your clear understanding of our Financial Policy is important to our professional relationship. Please ask us if you have any questions about our fees, Financial Policy, or your responsibility.
Full payment is expected at the time the service is rendered unless you have health care coverage through an HMO or PPO with which the physician participates. In this case, all patient deductibles, copayments, noncovered services and/or coinsurance are due at the time of service.
If a patient and/or guarantor cannot make payment in full at time of service, we will be happy to discuss appropriate payment arrangements.
As our services are provided to you, not to your insurance company, payment for the service is the responsibility of the patient. Therefore, all charges that are filed to your insurance company, and are not paid within 60 days from the date of filing, become your responsibility to pay.
